When you call to schedule your free consultation, our specialized Customer Service Coordinator will assist you in arranging a time that is convenient for you as well as send you a health questionnaire for completion prior to your visit.
When you come in, our Customer Service Coordinator can help you if you have any questions about your forms. He or she will create your file and notify the physical therapist that you have arrived.
The information you provide in the health questionnaire will allow our professional team of Physical Therapists to evaluate your needs and concerns during your free consultation. During your one-on-one consultation, you may be asked to perform certain tests/movements (lifting, stretching, pushing, pulling) so a plan of treatment for your recovery can be customized to your personal needs.
Next, the therapist will discuss the findings and plan a course of treatment. If it is found that you will benefit from physical therapy, you will be referred to either your physician or a physician that we deal with on a regular basis for further assessment. We can call at the end of your screening to book the appointment if you wish. (If you do not want to see an M.D. about this condition, please inform our staff and we can make arrangements for your treatment without a physician's referral.)
We will then send a letter to the physician informing them about our findings and recommendations so that they are aware of the situation before you arrive. If they agree with the diagnosis, they will write you a prescription for physical therapy which you can use for insurance purposes.
